This happens because there is one entry for each medium. For example https://musicbrainz.org/release/cc088bdd-5e4d-42ff-b348-df0937896185 has 15 mediums, so it's listed in the report 15 times. I'm not sure if this is the only affected report or other reports with relationships are affected too.
