Seems like it is not possible to add actions to the toolbar through plugins. In addition, it also seems like only toolbar actions can have keyboard shortcuts associated with them.

Currently plugins can add context menu items with accelerator keys but using them takes more mouse clicks / key presses than a toolbar icon with a proper keyboard shortcut.
