Plugin action MENU setting not honored in "Plugin Tools" on main menu bar

      When a plugin registers an action with a `MENU` setting to specify a sub-menu for the action, the sub-menu is not created for the "Plugin Tools" section of the main menu bar, but it is properly created for the context menus.
