The relationship dialog shows me no duplication warning and it even lets me submit a test edit:

Comparing the edit data of the test edit with the one of the edit that added the first relationship, shows that the link_order attribute is missing for the duplicate. I guess that's why it isn't detected as a such because I can't add a second duplicate.
