[MBS-2213] Auto focus for text fields

A lot of forms could give an auto focus to certain fields when being loaded. There's already a bug report for the inline search: http://tickets.musicbrainz.org/browse/MBS-1368

Other pages that come to mind:

Adding an annotation (focus to the annotation field)
"Tags" tab in a release (focus on the Update Tags field)
Create Relationship (focus on the Type drop down)
Edit artist (focus on the Name field)
Edit release group (focus.. on the Name field?)

A much smaller scope which I haven't seen mentioned above: I'd love to see that the login page come up with focus on the Username field. Today I have to hit backtab 16 times (or click with the mouse) before I can enter my username.
