Merging can fail if entities have aliases with the same locale

      Description

      Case study: http://musicbrainz.org/edit/17358970

      2 of these 3 entities being merged has an alias name="Danish National Symphony Orchestra" locale="English". This should merge into a single alias, but doesn't and thus we end up with 2 aliases with this name and locale, violating a unique constraint and failing to apply the edit.
