I often confuse certain kinds of "add entity" and "remove entity" edits, especially when similar edits are close by. The edit type in the title isn't sufficiently eye catching. It could be improved for example by color coding add edits as green as remove edits as red.
I find this most frequently problematic with cover art and relationship edits.
For example here there were two relationship edits right next to each other and I assumed the editor was adding both:
