There are bugs that make two tracks appear in the same position on the same medium (see http://chatlogs.musicbrainz.org/musicbrainz/2014/2014-04/2014-04-19.html#T20-41-58-844139). Adding a unique constraint for (medium, position) on track would catch these early.
