When a URL AR was duplicately added, there is a red warning and the submit buttons gets disabled.
I delete the bogus and then I have to manually remove the disabled attribute from the submit button.

At least I think it's how it happened several times. All I remember is that several times I was in front of a greyed out button and had to do this. I don't remember exactly when it should have been enabled that it didn't.
