Too many reports are submitted without URLs to the actual infringements, leading to me having to do additional work in digging out what I think the reporter may be referring to - or not being able to find it at all. Having some fields to add links to would remind reporters to actually link to the infringements, or examples of them.

Alternatively just some text on the /user/:user/report page, but I think text fields would make it more prominent.

It might also be neat to have a check that the fields are filled out, or that the Message text contains at least one URL and have the reporter verify that they want to send it if they haven't provided an URL.
