I would like to propose the addition of the following packaging types:

book: any bound set of pages or sleeves.
cassette case: a case designed to hold a compact cassette.

I would also add the following guidance to http://wiki.musicbrainz.org/Release/Packaging —

"If a release has multiple packaging types, use the outermost packaging"
