Make sure URLs are URL-encoded

      We currently have URLs stored both as URL-encoded and in unencoded UTF-8.

      This is inconsistent and means that the same URL can be entered in the database in more than one way, see http://bugs.musicbrainz.org/ticket/1102 and http://bugs.musicbrainz.org/ticket/3566
