Remove the "Add another track" button that is displayed at the bottom of each track list and instead use a small "add" icon at the top right of the track list in the medium format/medium title row.

The icon can be something basic like http://commons.wikimedia.org/wiki/File:Crystal_Clear_action_edit_add.png and the mouseover text should say "Add another track".
