Describe disambiguation comment field if no text has been entered

      The description "add a disambiguation comment" is okay, but once it's clicked it can be easy to forget just what that field was for. So if no text has been entered, or the text has been removed, a description should be shown.

      The description could just be "disambiguation comment" and to make it clear that it's just a description it should be shown in light gray.
